This full-time, permanent position offers the chance to provide skilled nursing care to clients while working in a supportive and compassionate environment.

About the Position:

As a Clinical Care Coordinator, you’ll play a crucial role in ensuring that seniors receive the highest quality of care. Under the supervision of the owner, you’ll assess clients’ physical, psychosocial, and environmental needs to create personalized care plans. This position involves critical thinking, leadership, and continuous evaluation of care delivery to ensure the safety, comfort, and well-being of each client.

Key Responsibilities:

• Client Assessment: Evaluate the health and environmental status of clients to develop and implement effective care plans.
• Plan of Care Creation: Utilize your nursing expertise to create personalized care plans tailored to each client’s unique needs.
• Medication Management: Oversee medication administration to ensure safety and adherence to prescribed treatments.
• Caregiver Communication: Regularly communicate with caregivers to modify care plans based on client progress and needs.
• Quality Improvement: Continuously improve the quality of care provided to ensure high standards of safety and effectiveness.
• Adherence to Policies: Follow Comfort Keepers’ established policies and procedures to maintain high-quality care.

Qualifications:

• Graduate of an accredited LPN/RN training program.
• Current registration with the Provincial College of Nurses.
• A minimum of one year of experience as an LPN or RN.
• Current certifications in First Aid and CPR (Standard First Aid).
• Proof of a negative TB test and up-to-date immunizations.
• Vulnerable Sector Police Check.
• Willingness to further your education (e.g., Footcare Course).
• Ability to work Monday to Friday.

Compensation and Benefits:

• Starting salary: $35/hour (reviewed annually based on performance).
• Additional pay: Bonus opportunities based on reaching quarterly company KPI targets, with potential earnings of $2,000 quarterly.
• RSP contributions: Company-matched contributions up to $100 per pay period, totaling $2,600/year.
• Vacation: 2 weeks (10 days) of paid vacation per year, increasing with years of service.
• Health and Welfare Benefits: 50% covered by the company after completing a 3-month probation period.

Work Schedule:

• Monday to Friday (40 hours per week)
